The goal is to increase confidence in fusionpbx, one step is to run by default with error_reporting (E_ALL ^ E_NOTICE);
...
So, please find in this set numerous small changes which eliminate numerous php warning messages.
Also, a small bug fix in an sql statistics routine, naming a count(*) field as count, so displaying correct detail.
If any of the proposed changes are omitted, the relevant page will experience php warnings.
There are no doubt fusionpbx pages I've yet to visit that still generate warnings, this set is nearly certainly not comprehensive.

Remove upgrade switch from advanced -> upgrade page. The work it did is handled by app/scripts/resources/classes/scripts.php and app_defaults.php calls it. So when running Advanced -> Upgrade -> App Defaults the scripts app_defaults.php will copy the scripts and rewrite the config.lua.

Another case where autocomplete creates a problem. New user account creation does not benefit from autocomplete. It assumes the user is the currently logged in user which is not what you want on a form to add new users. This code adds an autocomplete honey pot to defeat the browser developers poor assumption.

Browser developers disabled autocomplete in most browser. A move that makes an assumption that autocomplete is always good. In this particular case it creates a bug. There are a few legitimate reasons to disable autocomplete. In this case I'm disabling it as we are only updating the password when its provided by the user. In this case the user may be and administrator changing a user a password in this case autocomplete would offer the wrong password. Another case have two password fields that must match if both passwords are empty then the passwords are not updated allowing for other user settings to be updated.

Make the login.php use the correct app_languages.php file. Reason to do this is because login.php in the root is put in a position as being part of the core so I wanted the translations to be part of the core as well. And user settings is related to the user login. App/login is in the app directory to make the login and logout as an optional part of the menu since they could be incorporated in the theme.

Harden the security with specific permissions for domains, domain settings and default settings. Everyone using multi-tenant are encouraged to update. Run Upgrade Schema on wiki.fusionpbx.com. After updating go to advanced -> group manager click on the superadmin group and then select the permissions for domains, domains settings, and default settings. Logout and back in.

Add database driver to along with type it helps distinguish which type of database is being used when using odbc. Added app_defaults to core/databases so that the script config.lua can be created when the database is saved and upgraded. Previously it only saved a new config.lua when running upgrade or upgrade schema which was not intuitive.
